
Building capacity with tools and training to help families adopt healthier living
The Healthcare Provider (HCP) knowledge translation (KT) program was developed by London Health Sciences to help connect resources available through BC’s Live-5-2-1-0, a healthy living program, with urban and rural communities in Western Ontario.
This project is a collaboration between BC Children’s Hospital, London Health Sciences Centre (LHSC) and the Family Health Teams of Western Ontario.
Goals of the project
- To empower and support Family Health Teams to engage more families in healthy eating, physical activity, and wellness behaviours to help prevent Type 2 Diabetes.
Each toolkit item has been developed and tested locally, and shared with other Family Health Teams, who then can pick and choose the types of programs or interventions for their communities unique needs.
Accomplishments and Milestones
- London Health Sciences Centre adapted Live 5-2-1-0 for families in urban and rural communities.
- Family Health Teams around London were trained to use the messaging and tools in the Live 5-2-1-0 program
- 18 Family Health Teams in 4 regions have now added the program into their clinical practices.
